Somalia is rough, as are the rest of the countries on this list. But... in chaos is also opportunity! A small, tight, well-armed group can live pretty well, even become kings in their own right. The absolute power vacuum in much of Afica allows for piracy and bloodshed (duh), but as mentioned before, it also has mineral resources that can be exploited for massive wealth and power. While there are warlords, they are fighting each other for control. It would be rough, but not impossible, for a new one to emerge. All it takes is dedication and violence of action to stake your claim.
Now... North Korea and Myanmar, however, have none of those positives. Sure, you can live meagerly and in relative safety, if you do exactly as the overlords demand. But, you can never be secure. And you can definitely never rise to any level of success without following the Fearless Leader and the Junta. And even then, it is very difficult to break into their close circle. Not to mention that natural resources are not as plentiful, and the standard of living is nearly as bad as many African countries. So, I would say that it is pretty much a toss-up between North Korea and Myanmar as the worst place on Earth. But I would give Myanmar a slight edge.
